When a chiropractor subscribes to ChiroTV Network, they understand that ChiroTV Network is a subscription based service that uses the World Wide Web and streaming video to help them educate and market to their patients. This service is intended for office use and does not take the place of education and instruction by the chiropractor. The information provided on ChiroTV Network is for informational and entertainment purposes only. It is the subscribers responsibility to make sure that the content provided on ChiroTV Network is congruent with their practice and the educational material fits in with their scope of practice.
Health Visions, Inc. (producers of ChiroTV Network) assumes no responsibility for any misrepresentations or any information that the subscriber feels is not appropriate for their scope of practice.
The subscriber acknowledges that they are signing up for a monthly subscription service at the rate of $39.95 per month. They will be billed on a monthly basis to the credit card they supplied when they signed up to activate their subscription. It is the responsibility of the subscriber to keep their credit card information up to date to avoid cancellation of the service. The subscriber also understands that there are no contracts or set-up fees when they sign up.
However, there is a 30 day cancellation notice required by all subscribers if they wish to cancel the service. The subscriber will be billed one more monthly fee within that 30 day period before the account is completely removed from the service. All cancellation requests must be in writing or by calling Health Visions, Inc.
